Referrals

REFERRAL PROCESS:

Individuals with aphasia, family members, physicians, social workers, speech-language pathologists and other health care professionals can make referrals to the Aphasia Institute.

After the referral has been received:

  • The applicant will be placed on our waiting list.
  • He/she will be contacted by our intake staff within two months of receipt of referral.
  • The applicant will be offered an orientation session at which time programs/service will be discussed.
  • If they wish to proceed with the program, he/she will be offered a time for an individual assessment with a Speech-Language Pathologist and Social Worker.

Speech-Language Pathology Assessment

We ask that attempts be made to have a full assessment conducted by a speech-language pathologist prior to admitting clients to our program.

The report helps our staff to determine the appropriate group for the individual being referred. In addition, staff members at the Aphasia Institute rely on these reports as a baseline measure when a client begins the program.

Assessments can be arranged through the speech pathology department of your local hospital or Community Care Access Centre (CCAC). An assessment through a CCAC is typically arranged with the help of the client's family physician. Clients wishing to be assessed by a private therapist may contact the Ontario Speech Language Association at (416) 920-0361 for a list of practitioners. The Aphasia Institute can also provide a list of speech pathologists who provide private services. Please contact us at (416) 226-3636 for more information.

In the event that it is not possible to arrange an assessment, please call the Aphasia Institute and talk with the Manager of Intake and Client Services at 416-226-3636 ext.15. Every attempt will be made to accommodate the client who is unable to obtain an assessment.
