7. How do I purchase an ALA?

 

  1. Click here [http://www.aphasia.ca/orderforms.html] to access a Product Order Form
  2. Mail, email or fax your order with payment to:

  1. There are three payment options:
    1. Credit card – Include credit card number and expiration date on the product order form. If you are purchasing from the USA or overseas, please note that the currency conversion will appear on your next credit card statement
    2. Cheque – Send a cheque by mail with the order form. Please note, the cheque must be in Canadian dollars.
    3. Money Wire Transfer – Contact us [mbaldwin@aphasia.ca] to let us know which products you wish to purchase and we will create a Proforma Invoice. Please include this invoice number on all money wire documentation.  Please note, there will be an additional $20.00CAD required to cover the bank processing fees.
